Job Title:                                Family Support Specialist/Family Resource Specialist

Supervisor:                            Family Foundations Home Visiting Supervisor, Assistant Clinic Administrator

Summary: The Family Support Worker is responsible for initiating and maintaining regular (at least weekly) and long-term (up to five years) contact/support with families. This service position will follow the Healthy Families America model of care and use the Growing Great Kids curriculum.

Essential Duties and Responsibilities including the following. Other duties may be assigned.

Family visit will be home-based, family centered, and strength based.

Family visits will be directed at:

  1. Establishing a trusting relationship.
  2. Assisting in strengthening the parent-child relationship.
  3. Assisting with improvement of skills to optimize the home environment.
  4. Improving the family support systems.
  5. Increasing the family’s ability to problem solve and assume the role of advocate for themselves and their children.
  6. Identify other needs of the family, provide referrals to other services when necessary, and assist families to complete the referral/contact/appointment.
  7. Assist the family in establishing goals and a plan for accomplishment of those goals.
  8. Assess the normal growth and development of the target child.
  9. Completing data entry within 72 hours of family contact.
  10. Work as a team to ensure Medicaid Prenatal Care Coordination (PNCC) requirements are met as directed by the Registered Nurse providing PNCC services.
  11. Managing complicated schedules ensuring participation in all the necessary meetings and trainings including the weekly case management meetings, weekly reflective supervision meetings, monthly and quarterly grantee meetings.

Educational and/or Experience:


High School Diploma required.

Must be knowledgeable of the Bad River Reservation communities and culture.


Associates Degree in Early Childhood preferred and/or equivalent experience working in the field.

Other Skills and Abilities:


This is a required driver position.

Must possess a valid Wisconsin driver’s license and meet eligibility for tribal insurance.

Must have reliable transportation, and current insurance coverage.

Ability to attend trainings both locally and out of the area, and willingness to assist with program events on evenings and/or weekends if needed.

Strong interpersonal skills, ability to work as part of a team, as well as ability to maintain tolerance, patience and tact in working with variety of families of different backgrounds.


This position is contingent on ability to pass background check required. This position required for eligibility a Caregiver Background Check regarding working with children and/or elderly, and a required driver check, to meet eligibility for Tribal Insurance.

Language Skills:

Must have excellent written and verbal communication skills with the community, and internal/external agencies, as well as within the Maternal-Child Team.

The duties assigned to this position may involve bending, stooping, lifting, and carrying up to 25lbs. Work requires regular recurring periods of standing or walking.

Employees of the Bad River Health and Wellness Center must abide by the Bad River Health and Wellness Center Employee Health Policy. Employees must show proof of Tdap immunization or Pertussis booster, MMR and will receive TB test if employee has not had test within 1 year upon hire.
